The team registration should be made up to 15.03.2011.

1. Please download this file evs2011_registration_form.xls .
2. Fill the Registration Form sheet and the Team Registration Form sheet (both included in the xls file)
3. Send it by email to gdine@ine.pt.
4. We would like to have photos off all the players , so please send us by email to gdine@ine.pt one of the following:
   
   • a photo with all the participants identifying them all in the email, in jpg format, max size 800×600 pixels; or
   • photos of each participant, in jpg format, max size 127×144 pixels, name each jpg file with the player name.

The participation fee for the EVS weekend, 27 to 29/05/2011, is 210 Euros, should be payed up to 30.04.2011.

Includes:

• Two nights in the Praia Mar Hotel, including breakfast
• Friday
  • transportation from the airport to the hotel (2 buses, detailed information available after all registrations)
  • dinner Cocktail/buffet
  • welcome ceremony
• Saturday
  • transportation between the hotel and the playing fields

The organizer shall not be liable for any injuries or damage during the tournament weekend.
